I check the baby’s skin color not only on the face (which can be misleading) but also on the stomach, legs, and back. If bilirubin levels seem high (above 13–13.5), I may ask for a simple test to confirm. The ceremony will only take place once I'm sure it's safe. Eye Infections Redness, swelling, or strong green/yellow discharge in the eyes may indicate an eye infection. In such cases, both the Halacha and medical safeguards require postponing the Brit Milah until the infection clears, and in some rare cases, waiting an additional week afterward. If there is only minor discharge, I may suggest eyedrops to prevent infection from developing further. By alerting me early to any symptoms, you help ensure your baby’s readiness. For those who want only the general idea The Brit Mila is the removal of the foreskin from the baby’s penis, according to the commandment given to the people of Israel. A baby boy’s anatomy includes two layers of foreskin, called the Orlah and the Priyah. During the Brit Mila, both layers are removed so that the head (glans) of the penis is fully revealed, as required for a kosher circumcision. After the procedure, the baby’s penis will be gently covered with a protective bandage. This usually stays in place for about 24 hours. Healing is generally very quick, and babies return to feeding and sleeping normally soon after. From this point on, the head of the penis will remain exposed for life, a sign of the covenant given to Abraham and all his descendants as commanded in the Scriptures and described below. If that’s you, here is a careful explanation of what happens during the ceremony. To begin, it’s important to know that the Orlah (foreskin) is not the only layer of skin covering the glans. Beneath it lies another thin membrane called the Or HaPriyah (inner foreskin). This layer is not visible until after the initial cut. For the Brit Mila to be kosher and aesthetically pleasing, the Or HaPriyah must be folded back (and sometimes fully removed) so that the head of the penis remains fully revealed. This is an integral part of the traditional ceremony (mitzvah) as described by the Halacha. Now, step by step: 1. Sandak – Before the ceremony, the baby is placed on the Sandak’s lap. The Mohel opens the diaper and the Sandak gently holds the baby’s legs. Most babies cry already at this stage, mostly because they feel cold or the new position is unusual for them. 2. Sterilization – The area is cleaned and sterilized. This is completely painless and ensures the procedure is safe. 3. Hafrada (Separation) – The Or HaPriyah is usually gently attached to the glans. Using a special probe, the Mohel carefully separates it. This takes only a few seconds and may be uncomfortable. 4. Tfisa (Grasping) – The Mohel lifts the foreskin to the right position and places a metallic protective shield (Magen) to cover the glans. This guarantees that the glans itself is not touched during the cut. 5. Chituch (Cutting) – The Mohel recites the appropriate blessing, and afterwards the foreskin (Orlah) is removed in a quick and precise cut, lasting a few milliseconds. In my experience, this part is usually less painful than the Hafrada. 6. Priyah (Uncovering) – Once the cut is complete, the second layer (Or HaPriyah) is gently opened and quickly folded down, ensuring the glans remains fully visible. This step is essential for a kosher Brit Milah. 7. Metzitzah – Using a glass tool, the Mohel performs a suction over the wound without touching the baby. There are different traditional methods, and most mohalim use the mouth to do it, which I'm firmly against. This step is essential, but not all methods are equally safe. Unlike the standard rabbinic approach, I use a carefully adapted method that is both halachically valid and medically safer. If necessary, I’ll be glad to explain exactly how this works and why it makes a difference for your baby 8. Bandaging – The area is then carefully bandaged. The bandage protects the wound and holds the skin in place while it heals. Unlike many mohalim, I use a special powder developed in Israel, which works both as a natural anesthetic and as a very effective hemostatic (stops bleeding). This ensures a more comfortable recovery for the baby. It’s worth noting that different mohalim use different methods to stop bleeding. Unfortunately, some use substances that are not approved for neonatal procedures. Now you have enough information to choose a Mohel carefully, safe techniques, and a gentle touch matter.

Before removing the bandage There might be discoloration in the glans. It may look purple, but this is normal. This could be from the Mohel's grip. (Which is usually very strong). You should see a clean glans with a bandage wrapped around, right under the glans. If there is a little layer of film on the glans, this is just dried discharge from the newly revealed glans, and nothing to worry about. There will almost always be red staining on the diaper. This is because when the baby urinates, it causes the dried blood within the bandage to liquify, and it leaks onto the diaper. This is not bleeding. Bleeding is when you see blood actively dripping from the bandage. This is a pretty rare occurence. However, if this does happen, call the Mohel immediately. This is almost never a life-threatening situation, but it is important to deal with it as soon as possible. The first time the baby urinates after the Bris may be painful for him, as the urine will soak into the bandage and the urine will burn the baby a little bit. The Bris is not a comfortable experience for the baby, and if the baby is uncomfortable, you can give him medicine (Novimol, etc.) every 5-6 hours. Generally the baby should not react as though he is in tremendous pain. So if the baby is inconsolable, you should call the Mohel. The bandage may have moved or is too tight, and that may be hurting the baby. However, it is important to remember that babies cry before the Bris as well, and even baby girls cry, so don't get too concerned about this. You should check to see that the baby has urinated within 2 hours after the Bris. You can tell from the wetness of the diaper. If he hasn't, call the Mohel and let him know. Make sure the baby is eating regularly. Wake him up if necessary. If he doesn't eat, there is a risk of dehydration. Do not bathe the baby until the bandage has been removed. Do not touch the penis or the bandage. Other than that, the Mohel will have given you other instructions. Some will tell you to pour oil every diaper change, and some will tell you to smear cream on the bandage. It is important to listen to the instructions that were given to you, not what you heard from someone else, or what your relative heard from a Mohel in America. The reason for this is simple: Every Mohel does things differently, and because of this, if you don't listen to his specific instructions, it could adversely affect the healing process. For example, some Mohelim might need you to pour on oil, and if you put on cream instead, the bandage might stick to the wound. Or, some Mohelim might want you not to put on anything so that the wound dries out, and if you pour oil, you might cause bleeding again. And there are many more examples. From a Mohel's perspective, it is very bothersome when the parents don't listen to the instructions. I have had many times where the parents didn't listen to the instructions given to them, and things did not go as smoothly as they should have. This is a very common problem and should be avoided. After removing the bandage At this point, what you should see is an uncovered glans, with some red layer of skin surrounding beneath it. This is the Or HaPriya. Beneath that, (toward the baby's body), there will be an area which is yellowish. This is the wound area healing; do not try to remove it. It is healthy and important for the baby. Below that you should be able to see where the baby was cut. Once the bandage is removed, your baby should be more comfortable. Frequently, the area will look worse on the next days after the Bris. Do not worry about this. Also remember, red staining on the diaper is still normal. The baby should be bathed every day in warm soapy water. Again, follow the Mohel's specific instructions. He may tell you to pour on oil every diaper change for two weeks, or to put on antibiotic cream. Many mohalim instruct the parents to push the penis so that the glans is aiming downward toward the feet when closing the diaper. This can help it heal straighter.
